For years the industry measured safety by counting what had already gone wrong. Recordable injuries, lost time, days since the last incident. Those lagging indicators matter, but they share a fundamental flaw. They only tell you about failures that have already happened. By the time the number moves, someone is already hurt.
The problem with counting injuries
A low injury rate feels reassuring, and it can be dangerously misleading. Research into serious injuries and fatalities, often called SIF events, has shown they do not always sit at the top of a neat pyramid of minor injuries. A site can go a long time with few recordables and still be exposed to the specific conditions that cause a fatality. Counting paper cuts does not tell you whether the next dropped object or pressure release is waiting to happen.
This is why leading indicators have become central to modern HSE. Instead of asking how many people got hurt, they ask whether the work that prevents harm is actually being done.
What leading indicators look like
Leading indicators measure the safety work happening before anything goes wrong:
- Hazard identifications and near misses reported. A rising count often means people are paying attention, not that the site is getting more dangerous.
- Job safety analyses completed and reviewed. Consistent, quality analysis on high risk work is a sign the controls are being thought through.
- Corrective actions closed on time. Findings that sit open for months are exposures waiting to mature.
- Inspections and safety observations completed. Regular contact with the work is how conditions get caught early.
None of these require an injury to occur. They describe the health of the system while there is still time to act.
The catch: leading indicators need data you can actually see
Leading indicators only work if the underlying activity is captured and visible. If near misses are reported on paper and filed away, no one can spot the trend. If corrective actions live in email, no one knows which are overdue. The insight that prevents a serious incident is usually already sitting inside the organization. It just cannot be seen, because it is scattered.
